APPLICATION FORM UNDER THE PERSONAL DATA PROTECTION LAW

Use this form to exercise the rights listed in Article 11 of Turkish Law No. 6698. Using the form is not mandatory — a petition drawn up in line with the Communiqué on the Procedures and Principles of Application to the Data Controller is equally valid.

☐ 9. I object to a result produced against me solely by automated analysis of my data.

3. DECLARATION

I declare that the information and documents in this application are accurate, that the Company may request further information and documents in order to conclude my application, and that a fee set by the Personal Data Protection Board may become payable if the response entails a cost.

*If the data subject is under 18, the parent or guardian; if under guardianship, the guardian; or a person expressly authorised by power of attorney may apply, upon documenting that status.*

4. HOW TO SUBMIT

• Applicants applying in person must present identification. Please present only the front side of the ID copy (so that blood type and religion fields are not visible).
• E-mail applications can only be made from an address already registered with the Company.

5. HOW THE APPLICATION IS CONCLUDED

• Your request is concluded free of charge within 30 days at the latest.
• If the response entails an additional cost, the tariff set by the Personal Data Protection Board may apply.
• If your application is rejected, the answer is insufficient, or no answer is given in time, you may complain to the Personal Data Protection Board within 30 days of learning the answer and in any case within 60 days of your application.

6. NOTICE

This form is designed so that your relationship with the Company can be established and your application answered completely, accurately and within the statutory period. To eliminate the risks of unlawful data disclosure and to protect your personal data, the Company reserves the right to request additional documents and information for identity and authority verification. The Company accepts no responsibility for requests arising from inaccurate or outdated information, unauthorised applications, or problems in delivering our reply to the address you provide.
